Communication skills are vital for collaborating with interdisciplinary teams and providing clear explanations to patients and their families. Deep understanding of medical terminology, disease processes, and healthcare regulations ensures competent and safe delivery of care. Combination of clinical expertise, empathy, and adaptability to meet the complex needs of patients while upholding professional standards of nursing practice. Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The RN creates a caring environment focused on healing and wellness. Nursing practices are based on evidence, current research, hospital policies, procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinates, ensures, and provides direct patient care activities on the unit. Responsibilities include provision of nursing rounds, assessments, and treatments. Facilitates admission, transfer, and discharge activities. Provides patient- and family-centered education. Coordinates care and develops individualized treatment planning as applicable. Assessment of patient needs includes performing and documenting a comprehensive assessment in accordance with RN scope of practice and hospital policies. Considers patient culture, age, and developmental level. Identifies patient risk factors and presenting complaints. Obtains health information from family or others as necessary. Collects both objective and subjective data. Verifies initial admission assessment if obtained by other members of the healthcare team. Develops a problem list and prioritizes patient needs based on input from the inter-professional team. Designs actions and interventions relevant to patient culture and age. Determines desired outcomes in coordination with the patient and family per medical plan of care. Implements patient care therapies and procedures with empowerment and clinical autonomy within scope of practice. Explains procedures, treatments, and processes to patients and families. Initiates actions to alleviate patient stress and discomfort. Recognizes and communicates changes in patient condition appropriately. Conducts and documents ongoing assessment and responses to procedures and therapies according to established standards. Sets priorities for the care of multiple patients and assures resources to coordinate the plan of care. Organizes workload to utilize time and energy effectively relative to patient acuity and care processes. Delegates tasks appropriately and oversees team members in performance regarding patient care. Monitors the accuracy and completeness of delegated patient care provided by other team members. Identifies and addresses educational needs of patients according to prioritized problems. Facilitates the patient discharge process by executing discharge orders in a timely manner. Provides input into patient/nurse assignments based on staffing, unit activity, and patient acuity. Demonstrates active participation in unit-based activities in support of organizational values and priorities. Promotes a positive group dynamic and healing environment through words and actions. Ensures nursing excellence according to accepted standards, evidence-based practice, and a caring philosophy. Actively participates in department and/or unit quality improvement activities and core measures. Uses a holistic perspective regarding patient care.
